Le récit Morphine fut publié en décembre 1927, dans la revue Le Travailleur médical. En fait, le dessein premier de l'oeuvre est bien antérieur puisque Mikhaïl Boulgakov avait commencé à travailler sur ce texte dès la fin de 1917. Il songeait alors à écrire un roman qu'il voulait intituler Maladie. L'ouvrage devait rendre compte de l'état d'esprit d'un Boulgakov bouleversé par les événements révolutionnaires.
Certes il ne pouvait être question de publier un tel livre en Russie mais, en ces années-là, l'écrivain envisageait sérieusement d'émigrer. Ces circonstances expliquent sans doute pourquoi Mikhaïl Boulgakov revient en 1927 à un projet vieux de dix ans, elles expliquent pourquoi ce récit possède une telle intensité romanesque, un tel impact psychologique. Texte tendu à l'extrême, Morphine est aussi un véritable cryptogramme autobiographique où Mikhaïl Boulgakov expérimente pour la première fois cette structure en abyme qui sera magistralement utilisée dans Le Maître et Marguerite.
